What is on this site

Right now that is 46 nursing homes across Fort Bend, Brazoria, Galveston and south Harris counties, carrying 1000 health citations between them — 119 at immediate jeopardy, the most serious finding a surveyor can make. Every figure traces to the July 2026 release of the Medicare Provider Data Catalog, and the date is printed on every page.

Assisted living is different, and this site says so rather than papering over it. Texas licenses these facilities but publishes no star rating and no inspection score for them. So what you get here is the state license record — license type, capacity, whether the facility is certified for memory care, and who owns it. Where there is no number, this site shows no number.

Why this site exists

Medicare publishes an enormous amount about every certified nursing home: star ratings, payroll-based staffing, every citation with a scope and severity code, every fine. It is technically public and practically unreadable. A family deciding in three days about a parent who just fell does not have time to learn what "scope/severity K" means.

So we decode it. Nothing here is estimated, and nothing is invented — if Medicare did not publish a number, this site does not show one.
