The Texas complaint record for Houston-area assisted living

Texas publishes no star rating and no inspection score for assisted living. What the state does hold is a record of every complaint and incident it investigated — and it does not publish that either. We asked for it under the Public Information Act, and this is what it shows.

Where this came from

Nursing homes are federally regulated, so their inspection findings are published by Medicare and anyone can download them. Assisted living is regulated by the state, and Texas puts no equivalent file online. A family comparing two assisted living facilities has, from the state, a licence number and nothing else.

So on 27 July 2026 we filed a request under the Texas Public Information Act, Government Code Chapter 552, for the machine-readable complaint and incident history. The HHSC Long-Term Care Records Management Unit answered it on 21 August 2026 under reference A07292026.0450002, at no charge. This page and the facility pages under it are built from that response.

There is no feed to subscribe to and no URL to refresh. A newer window requires a new request.

What the record shows

Across the 163 assisted living facilities published on this site, the state investigated 2,619 allegations in 1,231 investigations between 23 July 2023 and 16 June 2026.

Substantiated is the state's own word: HHSC investigated the allegation and concluded it happened. We use it rather than a plainer synonym because it is the word in the record, and it is therefore the word that can be checked against the record. A substantiated allegation does not always produce a citation — often the facility had already corrected it.

The filled part of each bar is the substantiated share. A category with a long bar and no fill is one where families complained and the state did not substantiate what they alleged.

Who opened these investigations

This is the part most families do not expect. Of the 1,231 investigations in this window, 671 were reported by the facility itself, as state rules require, and 560 began with a complaint from outside. A building that reports its own incidents is complying with the law, not confessing to something — and a building with none on file may simply be one nobody reported.

Administrative penalties

Separately from complaints, HHSC may impose an administrative penalty — a fine. Across the published facilities, 26 of 163 have at least one, totalling 32 penalties and $78,650 imposed between 10 August 2021 and 7 August 2026.

The state's penalty extract carries an amount, a status and dates — it does not say what the penalty was for. So these pages report that a penalty was imposed and for how much, and never why. Do not read a cause into the number.

⚠️ How to read this, and how not to

An allegation is an accusation the state agreed to look into. It is not a finding, and it is not a violation. 2,268 of the 2,619 allegations here were investigated and not substantiated. Any count taken off this page without the substantiated figure beside it will describe something that did not happen.

A raw complaint count mostly measures how many people live in the building. A large facility generates more allegations than a small one for the same standard of care. That is why every figure on this site is shown against the total it came from, and why there is no ranking of facilities on this page.

Nothing on file is not the same as nothing happened. The state withholds investigations closed in the most recent 45 days, so the last few weeks are always missing. An absence in this record means no one filed or reported — not that an inspector looked and found nothing.

These are dated findings. They reflect the state of the facility on the day of the investigation, they may have been corrected since, and a facility can dispute them through the state's informal dispute resolution process.

Each facility's own complaint and incident history is on its page, with the allegation categories, the substantiated split and the investigation dates in its own context — which is the only context in which a number like this means anything. Start from the assisted living directory and pick a city.

What is still missing

The state supplied what was alleged and what it substantiated. It did not supply the inspector's narrative, so these pages cannot tell you what actually happened inside a building — only that the state investigated it and what it concluded. Individual investigation records can be requested from HHSC one at a time by survey exit date or case number.

We have not requested those, and we will say so on this page for as long as it stays true.
