Texas issues no star ratings for assisted living, and publishes no inspection findings the way Medicare does for nursing homes. The complaint and administrative penalty records above were obtained by public records request, not from a published feed. Citation-level inspection detail — what an inspector actually cited a facility for — is held but not yet published here. An empty inspection section is not a clean inspection record — where the state publishes nothing, this page says so rather than implying a finding.

Texas issues an Alzheimer's certification separately from the assisted living license. A facility without one may not hold itself out as memory care, so this list is the state's answer rather than the industry's.
